2018/2019 School Vaccinations

The Gallatin City-County Health Department will have appointment times available for school vaccinations throughout the year. Talk to your child’s physician to see if they will need any vaccines prior to school schedule pickup.

Please bring your child’s immunization record and insurance card to the appointment. Children with no insurance, or whose insurance does not pay for vaccines, qualify for vaccinations under the Vaccines for Children Program

2018-2019 Required Vaccinations

  • Haemophilus influenza Type B (Hib)
  • 1 dose  (given on or after the 1st birthday)
  • None Needed
  • Diphtheria, Tetanus, and Pertussis (DTaP, DT, Tdap)
  • 4 doses
  • At least 4 doses (one given on or after 4th birthday)


  • 1 dose of Tdap (prior to entering 7th grade)
  • Polio (IPV or OPV)
  • 3 doses
  • At least 3 doses (one given on or after 4th birthday)
  • Measles, Mumps, and Rubella (MMR)
  • 1 dose (dose given on or after 1st birthday)
  • 2 doses (first dose on or after 1st birthday)
  • **Varicella (chickenpox)
  • 1 dose (dose given on or after 1st birthday)
  • 2 doses (first dose on or after 1st birthday)

**All students must have evidence of immunity to chickenpox through one of the following:

  • 2 doses of varicella vaccine
  • Laboratory evidence of immunity or laboratory confirmation of disease
  • Diagnosis or verification of history of chickenpox or shingles disease by a healthcare provider (MD, DO, NP, or PA)
